Initiating Evaluation and Therapy services
Extra Steps Pediatric Therapies accepts referrals from parents, doctors, children’s developmental services, health departments, and other agencies serving children. Below you will find the information we need from you to get started.
For evaluations or therapy services, please call our office at 978-287-7878 or complete the online contact form here. Our Front Office Administrators will be happy to assist you in scheduling an evaluation date and time. They will also be able to assist your with basic information regarding the process should you have any additional questions. To facilitate the evaluation process, please email any reports that might be beneficial for your provider to have.
First Appointment
Once you have your appointment scheduled, please remember to complete the following information prior to arrival:
- Intake forms completed
- Referral and pre-authorizations obtained (if applicable)
- Registration forms complete
- Supporting documents emailed (preferred) or brought to appointments (IEP, Psychological testing, additional evaluations, etc.)
- Please bring socks
Initial evaluation at the first appointment is approximately 60 minutes. The therapist generally likes likes to speak with the family member as well as have time with the child.
