Direct Support – Independent Living Program

 

POSITION SUMMARY:
Ensures the highest level of excellence in care is provided to individuals with developmental disabilities. Ensures the provision of a safe and nurturing environment as well as opportunities for individuals to experience a variety of activities that promote community inclusion. FULL-TIME

 

RESPONSIBILITIES:

Providing individual instruction and skill development activities in the areas of personal care, meal preparation, household skills and community access.

Documenting progress through narrative notes and completing data collections sheets.

Providing hands-on instruction, verbal guidance, and close supervision in order for individuals to complete tasks and reach goals.

Support individuals in making meaningful choices about daily activities, meals, and leisure time.

Work with supervisors on issues relating to consumer plans, money management and unsupervised time.

Offering opportunities for the individuals to participate in a full range of community activities and to maintain connections with friends and family members.

Providing assistance and instruction to individuals in the areas of household maintenance and safety, yard maintenance and meal preparation.
QUALIFICATIONS:
High School Diploma/GED.

Experience in human services field a plus

Ability to write formal and informal reports and correspondence

Must be able to work independently while demonstrating effective time management, organizational and planning skills

Good judgment and decision making skills

Flexible, patient and dependable
Valid driver’s license and acceptable driving record.

Case Manager – Adult Residential

POSITION SUMMARY:

Works with Assistant Directors, the Director of Residential Services and House Managers to support assigned residences.  Uses teamwork and Positive Behavior Support as the foundation for effective functioning of the program to ensure that all individuals receive the highest quality of services possible.

Takes an active role in the development and improvement of the Residential Program and the agency as a whole with the goal of ensuring a best practices program and agency, including (but not limited to) the development and maintenance of new best practice initiatives and the development of new Residential programs.

 

RESPONSIBILITIES:

  • Works effectively and cooperatively with individuals, staff, families, community members and others; presenting self and Minute Man Arc in a positive and professional manner.
  • Focuses on ensuring consumer growth and development and consumer satisfaction as primary goals; regularly seeking feedback and input from persons served.
  • Supports the development and implementation of proactive skill development plans and interventions within a PBS framework, which place an emphasis on self-management and direction and increased independence.
  • Supports individuals to be fully integrated members of their community by offering frequent opportunities to participate in a range of activities, to make use of community resources, to make personal connections, and to be productive and contributing members of society.
  • Functions in a backup capacity for one or more House Managers in their absence.

 

QUALIFICATIONS:

  • Experience with adults with developmental disabilities preferred
  • BA/BS in social services field or two years related experience
  • Must be able to work independently while demonstrating excellent judgment, effective time management, organizational, and planning skills
  • Able to utilize a computer to maintain records – minimally must be able to utilize word processing and spreadsheet software preferably in Microsoft Office
  • Valid drivers’ license, an acceptable driving record and reliable transportation
  • Able to obtain, and maintain, certification in First Aid, CPR, Medication Administration, Drivers’ Safety, Human Rights, Fire Safety, and Universal Precautions
  • Ability to manage multiple tasks

House Manager

POSITION SUMMARY:

Provides day to day oversight of the operation and staffing of assigned residence. Functions as a role model and an effective leader for the assigned residence, using teamwork as the foundation for the effective functioning of the program. Maintains the clinical integrity of the program and ensures that the developmental, physical, emotional, and social needs of consumers are met within the context of a Positive Behavior Support (PBS) model.

 

RESPONSIBILITIES:

  • Ensure individuals are treated with dignity and respect in accordance with Minute Man Arc’s Human Rights Policy and DDS regulations. Ensures the agency’s Positive Behavior Support model is the guiding foundation for all services.
  • Demonstrate a high regard for each person and sensitivity for cultural values, expectations, and goals of individuals and their families.
  • Focus on ensuring consumer growth and development and consumer satisfaction as primary goals, regularly seeking feedback and input from individuals served.
  • Support individuals to be fully integrated members of their community by offering frequent opportunities to participate in a range of activities, to make use of community resources, to make personal connections and to be productive and contributing members of society.
  • Ensure individual instruction and skill development activities in the areas of personal care, meal preparation, money management, household skills, safety and community awareness and any other areas specified in the Individual Support Plan.
  • Provide effective personnel management and supervision of direct support staff, including training, staff meetings, payroll, and performance improvement assessments.
  • Provide on-call coverage and back up on-call coverage as designated.

 

DESIRED SKILLS/ATTRIBUTES:

  • BS/BA in a social service field or the equivalent of two years related experience.
  • Experience with adults with developmental disabilities preferred.
  • Experience leading/supervising staff
  • Knowledge of management practices including state and federal regulator regulations.
  • Strong planning and organizational skills.

Experience working with individuals with developmental or interested and willing to learn.

Valid driver’s license and acceptable driving record.

Positions are available full and part-time, with a variety of scheduling options: evenings, overnights and weekends.
